By car
Drive east on Eglinton Ave E to Don Mills Rd, then north across Lawrence Ave E and Hwy 401 to Sheppard Ave E. Or take the Don Valley Parkway north to Hwy 404 and exit at Sheppard Ave E, heading west. Then turn onto Parkway Forest Dr, then Forest Manor Rd, and use the free underground visitor parking.
By transit
Take TTC Line 1 north to Sheppard-Yonge station and transfer to Line 4. Ride east to Don Mills station, then walk a short distance to the clinic.
Tip: Bring your child's hockey or soccer schedule to the visit so we can talk about the right time to fit a mouthguard.
Unit 1 is outside the building at street level (not in the lobby). Free underground retail visitor parking. From Sheppard Ave E turn onto Parkway Forest Dr, then onto Forest Manor Rd.

Do you make custom sports mouthguards?
Yes. We take an impression or scan of your child's teeth and make a guard that fits their mouth. A custom guard is often more comfortable than a store-bought one, which can help kids keep it in.
What should I do if my child chips a tooth at hockey?
Save any broken pieces in milk or water and call us as soon as you can. We will try to see your child quickly and check the tooth and the area around it.
Is the DVP a good route from Leaside?
Yes, outside of rush hour. Take the DVP north to Hwy 404 and exit at Sheppard Ave E. Head west on Sheppard to Don Mills Rd, then follow Parkway Forest Dr to Forest Manor Rd.
